Email for sending Lab Orders

(Name of Patient on the Order) - I've attached the lab order form for you. Follow the instructions on the first sheet to find a lab near you to have the blood drawn, OR, as you were told in the office - you can just walk in to the High Point Lab near Bethany Medical at Skeet Club. To locate a lab draw center near you can use www.labcorp.com/labs-and-appointments to check online to find a lab near you and set up a time to have the labwork completed. Many of the draw centers are just walk-in, so you can make a time convenient for you. When you go to the lab, bring both sheets with you. At the present, we have primarily been working with labs in Greensboro, Kernersville and High Point for these tests. When a new lab was used for the first time - it can take longer than usual to process this information - since these tests are quite comprehensive. The lab where you go may have to make calls since the technician may be unfamiliar with how to collect for these tests, but in the past it has all worked out. On the sheet - it tells you to not bring your insurance card. DO NOT bring your card - DO NOT GIVE IT TO THE TECH!!! All the billing is sent to EVEXIA and our office, so you will not be charged for the lab work. Many times the technicians don't know how to enter the info without including insurance information BUT they must use our billing info - so do not give them your insurance card. If they have issues or don't understand - have them call us. If the tech cannot find "DOCTOR'S CHOICE - Clifton Mays" as the client - they can enter this as a "WILD CLIENT" so LapCorp knows where to send the results and blood samples. We also recommend this test be performed "fasting". So the results will be more accurate with 8 hours between food and lab work. If you have further questions, feel free to contact my office.
