chirotouch intake entry workflow 1\ demographics open chirotouch select the patient go to profile > demographics transfer all information from the front page of the intake form into the demographics section set preferred provider to mays, d c , dr mays set preferred location to hs triad, ll hp complete the referral category field click save add the patient’s emergency contact 2\ complaints go to charting find the patient by last name, first name select cart notes set the note name to initial / re eval / final eval hp under subjective , select intro new patient exam enter the initial dos click save open chief complaint answer all required questions click save add any additional complaints answer all required questions for each additional complaint click save 3\ review of systems select review of systems answer all questions click save 4\ medical history transfer all information from the history examination into health history , including surgeries medications illnesses accidents select family and social history answer all questions click save 5\ vitals select objective > vitals > ht wt pulse bp enter the following height in inches weight pulse bp systolic bp diastolic select left arm seated click save final check review all sections for completeness confirm all information has been saved correctly once all information has been transferred, write your initials on top of intake place the intake in dr mays’ box
