patient update is for any patient has not been seen in over 6 months, but less than 1 year after the patient has filled out the form, take the patient to exam room, take only their vitals and bring the patient update form back with you (dr mays only needs the re exam form) 1 in chirotouch find the patient 2 go to patient’s profile and select demographics 3 update any information given on form 4 go over to emergency contacts and update if needed 5 once all info has been updated write your initials on form 6 scan and upload into patient’s chirotouch documents and patient’s ohsc folder
