I have had the pleasure of evaluating Eugene Wood. I want to share with you briefly some pertinent information regarding this patient for your medical records. He presented to our office with the chief complaint of upper back pain and lower back pain. Following a consultation, I performed a physical examination including, orthopedic, neurological and functional evaluations. Current treatment consists of specific spinal manipulation and appropriate adjunctive physiotherapy modalities including therapeutic exercises. The prognosis at this time is fair. Please find the initial exam attached with more clinical information. I will be happy to keep you informed periodically about the progress and outcome at the time of discharge. We also now have the ability to take digital radiography in office and if applicable will have copies readily available for providers requesting records.
