new patient visit $87 (unless they are on established patient or a referral from another patient) • please give them a functional medicine intake to fill out while we wait on their bloodwork, letthem know we need it back at their earliest convenience (they can fax it or email to us, or drop it off) • bloodwork is $250 一 no exceptions, this must be paid upfront before we can order it • please create a new account for functional medicine ordering the bloodwork https //portal evexiadiagnostics com/account/logon login climay1! password (use current account password) 1 add the patient to the portal a login b patient > create new patient c fill out all the information first name, last name, street address, city, state, postal code, email address, phone number, date of birth and gender d save 2 ordering the bloodwork a lab orders b patient information> search by pid > enter the patient id# you created & search c click on your patient, their name and information should pop 니p d add recipients, very important step this will alow us to receive a fax notification e recipients, in the yellow column select fax f fax, type in 336 887 5710 g save, click ok, you should see the info under "add recipients" h next finalizing the bloodwork under diagnosis, select z0000 encntr for general adult medical exam w/o abnormal findings b under order codes, select 2366 731 evexia comprehensive bio screen panel (lc) c psc hold, may have to be pressed twice to confirm lab order should now be generate and you should save the pdf if the patient is in office, print it for them if the patient is not, email it to them and they must have this with them at all times when they go to get their blood drawn there is a standard email to send them use this standard email within the 7 business days dr mays should have recieved a notice that new lab results are available make sure dr mays has requested the functional lab report and prints it out for the patient call and schedule patient for follow up to go over lab results and set up functional wellness program
