NEW PATIENT
- Someone who has never been to any of our offices before
- Someone who has not been seen at any of our offices in the last 12 months
- Someone who is transitioned to another department of care (i.e. from Chiro to Functional Medicine)
REPORT OF FINDINGS
- Someone who has been processed completely as a New Patient and is coming in for their 2nd visit
- The Doctor will answer their most important questions on this visit:
1. What is going on? (what is the exact cause of their pain?) 2. Are we able to help them? (will our therapies help them get better?) 3. How long will it take? (# of visits, weeks they need to come in) 4. How much will it cost? (we go over their insurance coverage and their patient responsibility
OFFICE VISIT
- After their ROF visit, a patient will now become a regular office visit
- Only if they're a paying insurance or cash patient
- If they're involved in an auto accident, they will be a green personal injury visit
OFFICE VISIT ACN/PSF
- After their ROF visit, if a patient has United HealthCare, BCBS or Aetna, they may need a Clinical Submission Form to be approved prior to treatment
- They usually get a timeframe and which they will cover their visits
- For example, John may be covered from 8/1/17-9/1/17 and by using this visit code it helps us remember that his visits will eventually expire and we need to submit a request for more
PERSONAL INJURY
- If a patient comes in from an auto accident, every visit after their ROF will be labeled personal injury
- This helps identify the patient and their treatment status
- Visits for these patients need to be numbered based on their treatment plan given
RE-EXAM
- If a patient has not been seen in our office in the last 2-3 months
- If a patient is scheduled for a re-evaluation based on their treatment plan given
- If a patient has a new injury
LUMBAR SPINAL DECOMPRESSION
- This label helps us not overbook the decompression tables, so patients can receive treatment and allow a good flow
CERVICAL SPINAL DECOMPRESSION
- This label helps us not overbook the decompression tables so patients can receive treatment and allow a good flow
FUNCTIONAL MEDICINE/NEURO
- This patient is not being seen for chiropractic. They are here to seen about a Functional Medicine program such as Thyroid or Diabetes. Many chiropractic patients are part of the Functional Medicine program, but these visit do not include chiropractic care.
- This is also for our Peripheral Neuropathy Patients
WORKERS COMP
- This patient is being treated under a Workers Comp authorization
- Often times, they have a limited # of visits and we must keep up with them
DOL
- This patient is being treated under a Department of Labor authorization
- Often times, they have a limited # of visits and we must keep up with them
- These patients also get to see the Medical Doctor
VA
- This patient is being treated under the Department of Veteran Affairs
- They have a limited # of visits and we must keep up with them
- They need Authorization and the dates they have available visits. Make sure to put the total number of visits allowed in their current Authorization.
- On their last authorized visit, they must have a re-evaluation so that more visits can be requested.
MEDICAL DOCTOR
- These are patients who have paid the $75 consult fee to be seen by the MD
- These are also Personal Injury Patients being seen under HF Medical Lien
- These are also DOL patients
