Third page: This is your medical history. Please list any current medications and any known allergies to medications.

Financial Agreement: This form explains that we are in-network with your insurance and that you’re giving us permission to file on your behalf. (If not using insurance:) Even though insurance is not required to be a patient here, this form outlines our financial policy—please review and sign.

Final page (HIPAA Privacy Consent): This form explains your rights as a patient. Please print and sign your name where indicated.
