I want to share with you briefly some pertinent information regarding this patient for your medical records. He presented to our office with the chief complaint of lower back pain and left shoulder pain. Following a consultation, I performed a physical examination including, orthopedic, neurological and functional evaluations. Current treatment consists of specific spinal manipulation and appropriate adjunctive physiotherapy modalities including therapeutic exercises. The prognosis at this time is fair.
