Verifying UHC
Purpose
To verify patient eligibility, confirm in-network benefits, and determine chiropractic coverage and patient responsibility.
When
Use this process when a patient has UHC insurance and the office needs to confirm benefits before scheduling, checking in, or discussing payment options.
Who
This process should be completed by the front desk, insurance verification coordinator, or designated team member responsible for eligibility and benefit checks.
UHC DOES NOT PAY MORE THAN $55 PER VISIT - NEVER. IF THERE IS A DEDUCIBLE, PATIENT WILL ALWAYS PAY $55 UNTIL DED IS MET AND THEN WHATEVER % CO-INSURANCE.
Procedure
1.Log into UHC portal
Log into UHC insurance portal using office credentials
2.Go to Eligibility
Once logged in, go to Eligibility
3.Enter Patient Information
Type the following:
- Member ID
- Date of Birth
Then click Verify Eligibility.
4.Confirm Network Status
- Ensure all selections reflect In Network
5.Review coverage details
Acknowledge and review:
- Deductible amount
- Out-of-Pocket amount
- Important Note:
Once the out-of-pocket maximum is met, the patient is covered at 100%
6.Review Rehab/Therapy Services
Scroll down to COPAY & Coinsurance and click on Chiropractic.
7.Confirm copay, coinsurance, and visit limit
Under Chiropractic, review:
- Copay
- Coinsurance
- Visit Limit
7.Calculate Patient Responsibility
Example: 20% of $55 = $11 Co-Insurance
8.Print/Download and Save
Select Print/Generate a Reference Number and Save as PDF
